Ok on this note…what are the best examination features of severe AS? I’ll go first… I was once taught my someone ‘old school’ to feel for apical-carotid delay. I’d recommend it. If you can feel maximal apical pulsation BEFORE the carotid it’s always severe AS. Simples

RE the AS discussion (and to toot my flute)…this took A LOT of time to write…it was shredded several times by reviewers…but after much blood sweat and help from coauthors I’m very proud of it. Even though only about 10 ppl have read it 🙄😂

Completely agree. Underrated. Underused. Physiologically makes sense. Certainly the best data (in my opinion) for interpreting AS in what is a phenomenally challenging group of patients.

Love this…didn’t we use to call those ‘echo beats’…? P waves not regular because the retrograde ‘P’ resets the sinus node. Also the P wave buried in the terminal aspect of the third QRS doesn’t have the same morphology as the others: it’s negative in lead II (a pseudo ‘S’)!
#ECG OK, arrhythmia nerds - what exactly do you think is happening here? I'm not certain, but I do have a possible explanation.

1. Anterior MI (ST elevation on the ECG - not easy to see). 2. Complete heart block. 3. Apical rupture with haemopericardium. No LVH here - this is ‘pseudohypertrophy’ because the LV is so empty - not a surprise given the massive hole! Now…who’s for that beer at @BSEcho?
