This guidance is an important acknowledgement of the value of pragmatic RCTs to inform policy & practice. Academic coop trial groups have long done these RCTs to benefit patients, clinicians, payers, & regulators. Like us trialists, FDA is interested in seeing more of them.

We carefully assessed timing & features of ctDNA response and correlated with clinical outcomes in the first stage of BR36 published today in Nat Medicine. We use these results in Stage II to evaluate ctDNA to tailor treatment to improve survival of NSCLC patients.
In the first stage of the BR.36 adaptive trial in patients with NSCLC receiving anti-PD1 immunotherapy, the primary endpoint of concordance between ctDNA molecular response and RECIST response was met @ValsamoA @hopkinskimmel @CDNCancerTrials @JEDancey.
