In a new @Sensible__Med column, I had the chance to provide some much needed nuance regarding @RobertKennedyJr & the field of vaccinology. He is correct that most vaccines were never adequately tested for their overall health effects, so I hope this will change w his nomination.

I totally agree, I have seen no data to support splitting up MMR into 3 separate vaccines. It is one of the best vaccines out there, likely also having beneficial non-specific effects on top of the great efficacy against the 3 infections it is targeted against.

While these US vaccine recommendations are an improvement compared to the previous mass vaccination mess, I am very surprised that someone would even consider vaccination infants & toddlers <2 years of age against COVID-19. This is something that was NEVER recommended elsewhere.

Evidence-based rational vaccine policies are based on providing the vaccine where there is a net beneficial effect. By preaching mass vaccination when >99% of neonates, born to HBsAg-negative mothers, have no benefit (but risk of arm), your efforts are contraproductive & harmful.

Among the weakest arguments out there, made by @dr_andrealove, is stuff like this about how dangerous the infection is, & how easy you can get it. Hep B is very rare in high-income countries & vertical transmission (mother to child) is irrelevant when the mother tested negative.

Whether to provide hep. B vaccination at birth depends mainly on the local epidemiology of the infection, which is the reason why the vast majority of high-income countries only offers hep. B vaccine to neonates born to mothers that are HBsAg-positive.

Hepatitis B virus is transmitted via infected blood & bodily fluids through: • Birth • Unsafe medical practices • Close contact/living with someone infected • Sharing personal items • Piercings, tattoos, needles • Human bites It CAN be spread through sexual activity. 1/

The trial in question was rigorous and transparently conducted. BCG is cheap, safe & licensed - ready now to prevent neonatal sepsis & help curb antimicrobial resistance, incl. in crises like Gaza & South Sudan. It is therefore time that we harvest BCG's non-specific effects!

🌍Our suggested policy changes • End restrictive vial-opening rules • Give BCG immediately at birth, incl. preterm & very-low-birthweight infants • Track coverage by 1 week, not 12 months, and include BCG scar prevalence. These steps could save countless lives.

🦠 Strong anti-microbial signal: far fewer positive blood cultures - especially Klebsiella pneumoniae - after BCG. 💡 Greatest impact when given <24 h after birth, to babies born vaginally or in hospitals, to girls, and when mothers had a BCG scar.

In a new 🇸🇪 COVID-19 vaccine study of 3805 HCWs, between 12% (💉Comirnaty) to 48% (💉Vaxzevria and then 💉Spikevax) were affected by post-vaccination symptoms to the degree that they needed to abstain from work in the following day(s). It is crazy to think that this was mandated!

Scientific research that is novel and paradigm-changing is bound to ruffle some feathers, so bad faith anonymous attacks (rather than publishing a peer-reviewed response to the journal that published the paper), smearing at blogs, X or other SoMe, can most often be disregarded.

The best advice I can give is that of the Südhof lab: quietly check if the accusations are in any way reasonable (they most likely won't be), don't interact with these anonymous posts, and don't base your judgement of a paper or scientist on the number of random pubpeer comments.
