Drawbacks: not a quick entry although I have skipped rib removal during an emergency. Limited working space. No good self-retaining retractor. Not a good approach when ACC is a possibility to follow Operative Standards techniques or when multivisceral resection is necessary.

For your toolbox of approaches. Only us old folks remember this one.😉Open posterior ADX remains relevant when: an anterior approach will be difficult due to scar tissue, unfavorable anatomy for retro MIV approach, or bleeding needing conversion during posterior MIV ADX. @TheAAES
