Bonus: Have you ever wondered why the sartorius hurts so much when you needle passes through it? . It's not because it's a "sensitive" muscle, it's because those white lines you see in it are nerves to the sartorius. It hurts when you needle penetrates them. N. stim can prove.

6. Inject a bit, then double check what your pattern of LA spread is. Just seeing a single slice of ultrasound image can fool you. Always check proximal and distal to the needle view to ensure the pattern of LA spread is appropriate.

5. Use nerve stimulation in addition to ultrasound. 0.2 mA causing motor movement or tingling sensation is highly indicative of intraneural needle placement.

4. Know where the anterior cutaneous femoral nerve is. Sits in the fat layer just superficial to the sartorius. Looks like a little white ball.

3. Know where your nerve to vastus medialis is. At the start of the adductor canal, can be immediately lateral (separated by fascia) to saphenous n., or between sartorius & vastus medialis m.

2. Know where the saphenous nerve is in relation to your superficial femoral artery. At the beginning of the add canal, your saphenous nerve is MOST of the time lateral to your artery. It then crosses the artery medially (&superficially) as it goes down the add. canal.

For example, anyone can do an adductor canal block. But to minimize the chance of nerve injury, you need to:. 1. Know where in the adductor canal you are. Use the medial border of the add. longus meeting the medial border of the sartorius to inform your start of the add. canal.

The way nerve blocks cause nerve injury is via intraneural injection (of local anesthetic). I'd say to perform an efficacious block, people can do this after 40-50 blocks. But to perform an efficacious block while minimizing the chance of intraneural injection takes years.
