In summary: 11% of resections were futile High CA19–9, BR-GBC, and extended resections predict futility Nomogram helps preoperatively identify patients unlikely to benefit “Primum non nocere” — first, do no harm

🔍 Predictors of FS 1️⃣ log CA 19–9 ↑ (OR 1.12, p=0.014) 2️⃣ Borderline/Locally Advanced GBC (OR 1.97, p=0.001) 3️⃣ Extended or Multivisceral Resection (OR 2.01, p=0.014 Nomogram 👇🏽 Example: High CA19–9 + BR-GBC + planned multivisceral resection → ~33% risk of futile surgery.

⚠️ How common is Futile Surgery? 👉 133 of 1196 patients (11.1%) Median OS: FS = 8.9 months Non-FS = 88.7 months (p < 0.001) Even among recurrences, OS was worse after FS (6.1 vs 8.9 months). Slide summary👇🏽 @APHPBA @AHPBA @EAHPBA

Standard management protocol at TMH as per the TMH criteria by @DrVAChaudhari et al. Patients divided into early and borderline resectable GBC. BR-GBC - 344 (28.8%) offered Neoadjuvant treatment. Rest early GBC - upfront surgery

💡 Why this matters: GBC is an aggressive disease. Despite “curative” resection, many patients recur or die within months — questioning the value of surgery. We defined “Futile Surgery (FS)” as recurrence or death within 6 months after resection. @EAVegaMD @timpawlik @ItaruEndo
