2/ Observed PEP rate: 5 vs 9 events; 0.31 vs 0.6/100 PY; RR 0.56, 0.19-1.65 Sample size required at observed rates: >11084 What can one learn from a trial with total of 14 PEP events and 12 stroke/SE events over 3 years of follow-up?

I agree with @drjohnmโ€™s perspective on LAAC. As an additional piece of orthogonal evidence: Our group, as well as multiple others, have shown that AI-ECG can predict AF from sinus ECGs. When you apply these models or try to directly train an AI-ECG model to predict stroke, they

Great comments below: "but at this stage it seems appropriate to pause and reflect." Good luck with that Professor. I've been encouraging a pause and reflection on 5 continents, with little success. All of this could have been predicted from PROTECT and PREVAIL data.

Presented at #AHA25: In patients with atrial fibrillation after successful ablation, rivaroxaban therapy did not lead to a significantly lower incidence of a composite of stroke, systemic embolism, or new covert embolic stroke than aspirin therapy. Full OCEAN trial results:

Ok you all, here is my take of the CLOSURE AF trial. On @theheartorg https://t.co/WppsVncNfi Teaser: this may herald cardiology's biggest medical reversal. Nonindustry funded, non-biased trial. LAAC clearly inferior to best medical therapy (usually DOAC, but 10% no AC.) #AHA25

LAAC vs best medical care Thrombotic events - higher Bleeding events -- higher (counting procedure; also see AVERROES) CV death -- higher (please don't dare tell me that it was noise because if you do, let's relook at PROTECT and PREVAIL LAAC done in experienced labs
