An ICU rounds convo… Me: Can we reduce duration of ABX to 7 days? Them: Isn’t 10-14 more appropriate for gram (-) bacteremia? Me: Per the BALANCE trial, a 7-day course of ABX was non-inferior to 14 days. Them: Is this in guidelines yet? Me: … What would YOUR response be??

In a study of ~2.5M children, there was a small association between in utero Tylenol use and subsequent autism. But the association disappears entirely when you introduce sibling controls, implying it’s spurious.

Number of drug approvals is not a surrogate for improved patient outcomes. We don’t just need more drugs, we need drugs that improve outcomes. The pendulum had swung too far to approving any drug. Some restoring of balance is needed.

TRUST: I spoke of trust in public health during the pandemic a lot like the current health leadership in the Trump administration. However, there is trust and understanding what happened to stifle dissent & there is something else altogether which is assault on public health
