Our tPA trial for COVID is out! Thx to all! tPA bolus group with significantly improved oxygenation. 12 less days on vent (12 more VFD) & half mortality in tPA bolus group, signal persisted at each interim, need larger N to eval for significance. No ICH!.

On Monday we finished enrollment in our multicenter Phase 2a RCT "STudy of Alteplase for Respiratory failure in Severe acute respiratory syndrome coronavirus 2/COVID‐19" (The STARS trial, NCT04357730). study design described in stay tuned for results!.

great paper by @CareRutgers Sarah Orfanos, @JaredRadbel et al showing reduced dead space ventilation with tPA dosing in COVID ARDS! Response was best with early use after intubation and in those with highest levels of fibrinogen. great work!

Hard to understand why people are more comfortable watching the natural history of disease while awaiting RCT's to finish. It is a clotting disorder. if it weren't, why do otherwise healthy people show up with presenting symptom of thromboembolic stroke?.

Our response to ISTH guidance on COVID-19 coagulation management. Worth a read. In the absence if an RCT, the unknown has to be treated by clinical observation and clinical reasoning. Our take: anticoagulate early and aggressively. @BIDMCSurgery.

@US_FDA @SteveFDA applied 4 IND for tPA in critically ill COVID under Coronavirus Treatment Acceleration Program 8d ago, no progress, now been "reassigned" aka starting over, also told they wont review until week of April 27th!? CTAP program supposed to only take "days". Help?
