Don't get me wrong, I'm very much an 'actions have consequences' type of DM.
But if one of my players is arachnophobic and I CHOOSE to use a spider monster and turn that PC into a spider through my choices as a DM, then I'm the asshole there.
Consent is key.

In the latest D&D 5E adventure module, there is box text requiring the DM to get player consent before the character turns into a mind flayer after getting infected from poor choices the player made. Are there no consequences to poor actions in D&D anymore?

What are your personal worldbuilding turn offs.
The ones where there's nothing wrong with it, it just isn't your cup of tea.

Mine is that not enough players respect their Dungeon Masters. They do an insane amount of work to prep and run the game for folks to just hop in.
If your table goes the extra mile, hell yeah! But I've heard so many horror stories.
