New data on the primacy of signaling in the residency application process. Note the massive drop-off in interview rate for signaled/non-signaled applications, especially in ‘big signaling’ specialties. Deciding where to signal is *the* new high stakes meta-game for applicants.

2) Look for the position of the anterior ethmoid artery. Usually within the skull base and protected during FESS, but can be low-hanging within the ethmoid cavity. If injured, the vessel can retract into the orbit causing a retrobulbar hematoma - vision threatening emergency

Nice summary, two comments from the surgeon perspective: 1) Emphasize position of uncinate relative to orbit. This is most worrisome in silent sinus syndrome where the orbit is also inferior displaced and radiographically dehiscent, increasing orbital complication risk

1/Time to FESS up! Do you understand functional endoscopic sinus surgery (FESS)? If you read sinus CTs, you better know what the surgeon is doing or you won’t know what you’re doing! Here’s a thread to make sure you always make the important findings!

📣New data! High signaling approach reduces residency #ApplicationFever. Applicants don't need to over apply to maintain visibility to their choice programs. Programs benefit by getting more time to perform holistic reviews. #otomatch @headmirror_com #MedEd @AAMCtoday #ERAS

Great to see preference signaling successfully implemented and studied in many specialties. While signal approach (# & tiered) varies across specialties, so do application dynamics. Continued collaboration and investigation helps all understand, customize, and optimize

Preference signaling for radiology residency applications helps a lot, but where do you send your 6 gold and 6 silver signals? Typical allocation in our survey: 2 gold→aspirational/reach programs 3 gold+3 silver→likely matches 1 gold+3 silver→safeties https://t.co/70L78ZZ6Ii

🧵Observations on #Otomatch2024. 1 or 3. Big win for #BigSignaling. Providing ENT residency applicants with a large number of preference signals (25) drives down #ApplicationFever [25%⬇️number of apps sent per applicant, 30%⬇️apps per program] while filling 99.7% of all positions
