Was great to present my work at #ICOSET . The feedback from trainees and trainers was telling - we need to be talking about this more!.

Another great #ICOSET @RCSEd talk. Trainers value enthusiasm, professional behaviour & taking responsibility . But need time, resource & support. Survivor bias amongst trainers & they train as they were trained . No incentive to be a good trainer nor change training

I genuinely want to know what surgical reg is going to agree to a locum at this rate? Surely the only outcome from this is the shifts don't get filled- so is this the intention? @wannabehawkeye.

These new extra-contractual rates in @uhbtrust are abysmal. The average hourly rate for a plumber is £50 per hour. For an emergency plumber it is £112.50 with a £110 call out fee. Massive withdrawal of locums is the only answer to this.

If you know this information, and you still make pregnant trainees in your programme do nights/on calls/unadjusted hours the SECOND you find out they are pregnant, your're not just part of the problem for these complications, you are directly responsible.

Up to 82% of pregnant surgeons experience miscarriage, preterm delivery, and intrauterine growth restriction. Occupational hazards, including prolonged standing, physical exertion, overnight shifts, and teratogenic exposure, contribute to these risks.

And I'm not talking about heads of college or deanery's. Im talking about consultants in the job. How many of you have finished a ward round, then turned around and asked your team for feedback on it?.

If you want change and improvement in the workforce, you have to encourage it throughout the workforce. If you want to be a dynamic "disruptive" leader, then you need to encourage feedback and criticism and engage with it.

How many times have we seen "leaders" not rock the boat or challenge the status quo? How many have we seen strive for the position, only to do nothing with the responsibility?.

Roughly most people spend between 5-10 years as a trainee. Why do we think people can automatically unlearn submissive behaviours once they are in positions of power.

We are taught to not rock the boat, not to question those immediately above us in power structures, and more importantly " grin and bare " bad training and situations, and if you want to change things wait until you have positions of power.
