Great discussion of issues around MCID for study planning. One can reach markedly different conclusions about what constitutes the smallest relevant effect based on whether the trial purpose is for drug/device approval or policy making vs informing patient choice.
I think the biggest argument against MCID is that it’s not a fixed quantity, and it’s a premature forced decision made by the trialist(s) which should be made at the time of decision making (point of care) by the treating clinician. From Berger’s text:

Finding effective ways to prevent & treat pain during cesarean delivery is a key way that pragmatic trials can make care better. I recently shared some thoughts in @IARS_Journals about how we help maximize the benefit of such trials for our patients:.
@HofkampMichael @rachesomerstein @SOAPHQ @ASALifeline I agree with you re: need for new ideas. “Do it the same way as always, just do it better” is not a realistic solution. RCTs can help define what approach is best, but it will take bold leadership from OB Anes community to overcome barriers & address the important questions.
