@GlobalSurgeryJC Patient based advocacy is extremely critical due to their lived experiences which can inform us how to improve their care.I think patients as stakeholders will help health financing mechanisms move towards making decisions that patients will be more receptive to

@GlobalSurgeryJC Some topics to consider here can be: 1⃣ Concentration of #resources and #healthcare facilities in urban areas, leaving vulnerable and #rural areas apart 2⃣ #Quality of #road and also #RoadSafety 3⃣ Focus on railway transport, but not river transport or other modalities

Q4 The 2 hour threshold for travel to a CEmOC has been proven virtually impossible in LMICs. What role can HICs play in helping to make the 2 hour mark more achievable? #GlobalSurgery #GlobalSurgJC #PedSurg

@GlobalSurgeryJC In this regard, #ethics is crucial. The American Surgical Association Working Group on Academic Global Surgery suggests some ethical principles for partnerships: 1⃣ Focus on #local #surgeons and #patients priorities 2⃣ New collabs should not destabilize previous programs

@GlobalSurgeryJC 3⃣ #Capacity building: minimize the risk of #braindrain & avoid emphasis on costly technology 4⃣ #Research: obey rules of fair #authorship and collaboration 5⃣ Visiting #surgeons should adhere to the same standards for clinical care like at home @aaglobalsurgery @AcademicSurgery

@GlobalSurgeryJC Short answer: None! Long answer: HIC should not automatically assume they can help solve all LMIC problems (sometimes they make them worse). Once a problem had been identified, challenge communities to come up with their own guidelines for context-appropriate prehospital care

@GlobalSurgeryJC Models should consider 'time' especially in cities...for example, the same trip at rush-hour might take 3-4 times longer than the same trip at midnight

@RaymenArviin @GlobalSurgeryJC Great point, LMICs located in areas with more pronounced seasonal changes also have to contend with weather as an impediment to travel.

@ShahnurShah @GlobalSurgeryJC Another excellent point. Quality and size of road infrastructure are often neglected caveats when estimating time of travel.
