🧠 Take-home:.JAK-STAT drives oncogenesis. JAKi may reduce some part of oncogenesis but impair NK cells. Cancer risk higher vs TNFi, not vs placebo or methotrexate.

🧪 JAK inhibitor selectivity isn’t straightforward. They may prefer one JAK, but:.– Selectivity is dose-dependent.– Effects are pleiotropic.So it’s hard to predict the exact immunological or oncological impact of each JAKi.

But… in a network meta-analysis: .➡️ TNFi may lower cancer risk vs placebo.➡️ JAKi might be neutral

So what about JAK inhibitors in rheumatology?.🧬 They block the same pathway but.🧪 ORAL Surveillance: tofacitinib vs TNFi in RA.➡️ Higher malignancy rate with tofacitinib.➡️ Mechanisms may relate to NK cell inhibition (esp. via JAK3/STAT5)

JAK inhibitors are used as a treatment is some haematological malignancies. Inhibition of this pathway is even being explored as adjunctive therapy with immunotherapy. #EULAR2025

In solid tumours, aberrant JAK/STAT signalling plays a role in hepatocellular, breast, and prostate cancers—driving immune evasion, metastasis, and lineage plasticity.

We began with the biology: the JAK/STAT pathway is involved in cellular proliferation, inflammation… and cancer. JAK2 mutations drive some myeloproliferative neoplasms. STAT3 and STAT5 promote oncogenesis; STAT1 may be protective.
