Does anyone still think we need a PVI ( PFA) vs Sham ablation study ?

The use of pLVADs during VT ablation is associated with longer procedures and higher procedural complications without any benefit in acute or long-term outcomes.

‼️Featured in JCE‼️ Pentaspline PFA lesions can vary by pose and location. Despite non-rotational applications, PV were acutely isolated after PFA. PLA 🌸 applications were irregular, with minimum diameters of 14.9 and 17.7 mm. Roof lesion footprints vary widely. @jskoruth

CTI with flower pose- Not the wide lesions.... that's also how wide the impact on the coronary can be , also based on contact the width can be variable.. Congratulations to Dr Ariel Banai for putting this togetehr

5)Significant variation of flower pose lesions on LA roof: Contact even more important here (Fluro can be deceiving)- but remember its a big footprint so your lesion can extend quite anteriorly

Remember- forceful contact gets better lesions- but if the tissue stretches the footprint will be smaller than the catheter

3)PV basket pose lesion regression occurs with single more than double applications: even double apps regress ! redundancy is key to durability 4)Posterior wall flower pose lesion regression can occur even after 2 apps and lesions can be smaller than catheter diameter

What does this paper demonstrate 1) Unreliability of acute PV isolation endpoint with PFA; - OK so every one already knows this 2) Band-like lesion footprint of the basket pose : this is important so we know how to position the basket to ablate the entire sleeve
