Thank you all for voting and reposting. I stopped cangrelor, inflated a 4.0 mm 🎈 in-stent (for 45 min), and called my neuro colleague to have coils ready. Protamine was too risky with gear still inside. After a few minutes—no more pericardial leak. Uneventful recovery.

🔎 Pneumopericardium can occur during pericardiocentesis even with slow drainage if >1L is removed. Sudden pressure shifts allow air entry. Air can be drained if the catheter is still in place. #Pericardiocentesis #CardioTwitter
