Important that this is NOT a trial of bicarbonate concentration. It’s a trial of veverimer. It didn’t work. But the fact that it didn’t should lead us to question the practice of chasing bicarbonate numbers in general.

Lots to unpack for VALOR-CKD. Why the active run-in & enhancement? Probably selected for less severe acidosis. But many hypothesize that acidosis in this range promotes progression, and study seems pretty conclusive that veverimer does NOT prevent progression in this population.

Great to see rigorous RCT evaluation of e-alerts by @fperrywilson of Yale and @KPMProject . They CAN influence drug prescribing in AKI. Nice discussion of how this can and should move forward - this a nice step.
