Although donor-derived HAV infection is a rare event (only 2 prior reports in the lit.), it may be under-recognized due to the self-limited nature of hepatitis A. However, given the marked increase in HAV in the US over the last decade, this risk may be increasing. fin/.

The best preventative measure is to make sure transplant candidates (and recipients) have received HepA vaccination. As always, it's better to vaccinate BEFORE transplant than after. 9/.

Lab-based screening for HAV is unlikely to be useful because:.1. PPV of screening anti-HAV IgM in only 10% in asx ppl.2. Mean HAV incub period = 28d (15-45d). Donors with acute HAV may be infectious but undetectable during this window period.3. No commercial HAV RNA test.8/.

The recipient of the donor liver had evidence of immunity to HAV and did not become infected. 7/.

Fortunately both kidney transplant recipients made a full recovery with supportive care. HAV was detectable in serum +/- stool 176 and 189 days post transplant in the two recipients. 6/.

The recipients developed symptomatic hepatitis A 19 and 75 days post-transplant with high HAV VLs detected in serum and stool. Banked donor serum also proved to be HAV RNA positive (2.6 million IU/mL). Genomic analysis showed that all were infected with the same HAV strain. 5/.

* The donor had potential risks for HAV exposure and reportedly had a recent fever but had no GI issues, jaundice, LFT abnormalities, or other signs/symptoms of acute hepatitis A prior to organ procurement. 4/.
