FYI; I'd highly recommend people also look at the Larson '21 paper from @GrailBio , another good cfRNA report - but I never saw Grail do anything with it.

It is not clear if dual extraction from a @StreckLLC tube would yield ctDNA and ctRNA at levels that would mean both analyses could be completed from a single tube, or whether a specific RNA tube would need to be collected.

It's an intersting read and combined cfDNA and cfRNA methods are available to us e.g. through Caris Life Sciences. but we'll need to be collecting plasma in the right tubes!.

Applications in other settings: The paper does not mention HER2 signature detection for ADCs. Or applications in ER or AR driven breast and Prostate cancers.

In n=10 patients they demonstrate that RARE-Seq can detect a METamp Sig in 2 of 3 MET amplified NSCLCs, EGFR C797Sm in 2 of 3 confirmed C797Sm patients, and histological transformation via a SCLCSig in 3 of 4 patients with biopsy-proven histological transformation (see Fig. 4b).

NSCLC (mainly stage IV LUAD) plus 50 healthy controls showed RARE-Seq detected differential expression of 96 genes overexpressed in LUADs. LUAD signature analysis had 73% sensitivity at 95% specificity for LUAD detection (mostly Stage IV).

They showed that platelets partition with plasma during blood sample processing and confound cfRNA analyses, which they fix by blacklisting platlet transcripts (at least I think this is the simplest explanation of what they do).

They use targeted RNA-Seq (5000 cancer loci + 50 housekeepers) and say they get 50-fold better sensitivity of detecting cancer-associated cell-free RNA in plasma approximately compared to cfRNA-Seq, which holds promise for non-invasive disease monitoring and gene expression.
