Minor point but how can you do lucent line implant without first resolving the lucent line on your valve?….We find you often need to modify fluoro working view away from 2cv by a few degrees to achieve this. Then can do LL implant all cases
Is fluoroscopic Lucent Line (LL) reproducible in S3 #TAVR? We report the largest series >1100 pts showing it’s NOT and in fact didn’t ⬇️ pacemaker or implant depth and no ⬆️⬇️ in PVL. Why? Bc you don’t see LL all the time on 3-cusp view or even cusp overlap view. Once deployed LL

answer is you rarely need to try & if you try you should be in >99%. But if valve is in body >30mins skirt will enlarge, if valve gets pushed or pulled onto stopper balloons it may enlarge. You can check for this by X-ray. If any Of above. Deploy in safe spot distal to arch

How easy is it to remove large Octacor 32mm #tavi from python sheath? We could not cross the arch but we could not remove it- with cells of valve pushing out of the expandable fold of the python. Surgical assistance and able to complete procedure- any experience or data??

#tavr #tavi fracture of an evolut with early failure successfully treated Lovely ct images and planning
Published today @JACCJournals a rare finding of Evolut TAVR frame fracture after post-dilatation, within a prior surgical bioprosthesis. This resulted in early SVD with torrential AR. Successfully treated with TAV in TAV in SAV. Looking forward to present it at #NYValves2025

Treating more patients with life threatening disease both in cath lab & theatre. We ‘blew the whistle’ on our waiting list and (to a certain degree) solved it while maintaining quality and training . Not sure how that can be a bad thing 🤔

Set up by cardiac anaesthetist. Outcome for nurse same anaesthetic (albeit selected). TAVI fellow 400 cases last year, PCI fellow 1000. Key is treating more patients, AS has a mortality 2-4% per month on a waiting list. So more SAVR, more TAVI, more training=triple win

Is a TAVI a particularly straightforward procedure to sedate for? Are the complications low? Did an anaesthetist suggest they weren’t required?

Tremendous achievement this innovation has probably done more to facilitate patient access and treatment delivery. Well done all - looks like I missed a cake…😒
In September 2022 we tweeted our 1000th nurse-led sedation TAVI case. Today, we did our 2000th. We are hugely proud of our nurse-led programme which continues to go from strength to strength. Congrats to the whole team.
