مما يرسخ خبرات الطبيب ويعطيه القدرة على علاج الحالات الصعبة كثرة معاينة المرضى إذا كان موظف و المنشأة التي يعمل فيها تعاني من سوء الإدارة تصبح العيادة شيء مكروه للطبيب فلا يتطور ولا ينتج معرفة جديدة المفروض (استشاري) = يعطي استشارات ويعاين مراجعين باستمرار ولكن هذا ليس الواقع
