@DrTrishaPeel @Monash_FMNHS @peripatetical @nhmrc Among all patients, the addition of vancomycin was no better than the traditional cefazolin antibiotic. In patients undergoing knee joint replacement, the risk of infection was higher in the vancomycin group, 5.7%, than in the placebo group, with 3.7% infection rate.

@DrTrishaPeel @Monash_FMNHS @peripatetical @nhmrc The ASAP Trial looked at 4239 patients undergoing hip or knee joint replacement in 11 hospitals across Australia, including regional and private hospitals. Half of the patients got vancomycin + cefazolin, the other half got placebo + cefazolin.

@DrTrishaPeel @Monash_FMNHS @peripatetical Enter @DrTrishaPeel with the Australian Surgical Antibiotic Prophylaxis (ASAP) Trial - a pragmatic RCT of combination vancomycin and cefazolin surgical antibiotic prophylaxis. ASAP was fully funded by the @NHMRC for $3.5M from 2017 - 2021.

@DrTrishaPeel @Monash_FMNHS Prof Allen Cheng (@peripatetical) says many centres in Australia & globally have adopted the practice of giving both cefazolin & vancomycin, despite the lack of clear benefit. Vancomycin is better against MRSA but not as good for MSSA/MSSE, and is associated with some toxicity.
