This deeply resonates with me. The term clinician educator, in many circles, has been equated to “clinician publisher.” We are loosing great diagnosticians and humanitarians in academic medicine to the publish or perish model. A thread of thoughts below: 🧵

#Inthetrenches. I use this phrase often in the cases I post. Happy to read this piece from Dr Hilal highlighting the role of clinician educators

Overview of complement profiles in GN. As expected, C3 glomerulopathy had highest spectral counts of all complement proteins. Surprisingly, monoclonal Ig-associated GN including MIDD & PGNMID had next highest complement burden. C4A & C4B pathways in GN https://t.co/vLbHHjcHZc

1/2 NON-ANEMIC IRON DEFICIENCY There is actually a literature, including Cochrane reviews, that questions whether to treat such patients. Really?? So you are sitting there with ZERO iron stores, on the way to iron-deficient erythropoiesis and anemia.

Look at the twitter pages of some of the biggest medical centers, and you’ll see so many boast #USNWR ranking. The rat race is real and sad.

The point is, as long as we value the USNWR rankings, hospitals (and schools, and everyone else) will chase the metrics. Even if chasing them generates adverse environmental effects or costs money that could have been more compassionately spent elsewhere (e.g., patient care).

Scientists spend too much time taking “professional selfies”. A paper from PNAS describes what it takes to be a scientist today. Key points (that I always emphasize myself): #AcademicTwitter @AcademicChatter #research

To those who insist on using Hct over Hb to describe a patient with anemia (I hope they are few in number), does this patient have anemia or not?

This study found hospitalized patients with acute coronary syndrome are less likely to receive clinically indicated invasive cardiac care such as angiography if they have chronic kidney disease. Not receiving such care is linked to higher 6-month mortality https://t.co/YddxfN8dPB

My point is this: Those of us involved in residency selection need to be sure that the competitions we push our applicants into result in them acquiring the skills that we really care about. Then, we can have a virtuous competition instead of a senseless arms race. /end

Why do elite colleges pump students into the same five industries: law, medicine, tech, investing, and management consulting? For answers, let’s turn to the philosopher René Girard.
