We quantify successful coverage by PTV_opt coverage since we are adapting each day. Mandate that PTV is covered by 25 Gy (otherwise TPS will push most of low dose away from OAR as well). Recognize that not everyone has access to adaptive radiation therapy. 3/.

We evaluated 48 patients treated with MRgART. 79% abutment, 21% invasion. Dose 50 Gy in 5 fractions. 1-year LC, PFS, DMFS and OS were 85%, 70%, 70% and 63% for this high-risk population. 2/.

Congrats to @AldenDSouza3 @KylieKangMD for this work on MR guided adaptive SBRT for pancreatic tumors abutting/invading organs at risk in @IJROBP. Classic teaching is to prolong fractionation and avoid SBRT for OAR abutment/invasion due to toxicity. Still true in ART era? 1/

We now assess CBCT simulation on Ethos prior to treatment. If significant air artifact then patient is prescribed 20 min supine positioning prior to each adaptive fraction. Significant improvement in QOL of adaptive team, contour accuracy and contouring times👏 4/.

With @VarianMedSys HyperSight (6 second CBCT) this effect may be less. However, if you're noticing air artifact or would like your on-table imaging to improve try this out! We ask patients to lay supine for 20 min on hospital bed then slide/transfer them over to tx table. 3/.

We scored 1644 CBCT images (104 CBCTs, 52 fractions, 26 patients) on a scale of 1 to 4. 1 = no artifact vs 4 = 🤬to contour for adaptive. Many more 4s in the initial CBCTs. Hypothesis is that the bowel settles during that 20 minute period. When settled, less air artifact! 2/.

Now available online, adaptive / online imaging hack to improve your on-table image quality! . The story begins when we noticed that our CBCT images for adaptive were consistently better with verification (after adapting) than the initial (used for contouring). 1/
