2⃣ Standard hb threshold for transfusion is 70 g/l BUT rapidly falling haemoglobins should be rechecked at an appropriate interval. New ⬇️hb and ⬇️BP is bleeding until proven otherwise even if you can’t see the blood🩸🩸🩸.

But note, patients with a previous transfusion sample only need one sample this time (because we've already got a result to compare today's to). Save your time by knowing when a second sample is required! ⌛️.

The risk is that bleeding the wrong patient ➡️wrong ABO group on testing ➡️ABO incompatible transfusion ➡️haemolytic transfusion reaction and death.

1⃣ The reason we need two samples prior to releasing ABO-matched blood for transfusion to a new patient is to confirm the correct patient has been bled and thus the ABO group is correct. Never take the two samples at the same time. 💉💉.

Identification is critical for patients and healthcare workers alike. We were talking about Dr Granger’s work in a transfusion meeting a few weeks ago. Her legacy is so embedded that some HCPs don’t realise it ever wasn’t a thing.
Today we remember the one and only Dr Kate Granger MBE who died on this day in 2016. Kate was an inspiration to thousands across the NHS and her legacy lives on in so much of what we do. 2023 marks 10 years since she launched the #HelloMyNameIs campaign.
