Done with 5/6 sub Is and let me tell ya, if gen surg is your calling a grueling line up of sub Is is a right of passage. You learn SO MUCH and it tests your commitment to your specialty.
The result: I’m more excited than ever to be a gen surg intern. Bring it on.

4:30 wake ups (if I’m lucky), 12-14 hour days 6-7 days per week, 20 hour road trips across the country, and tons of OR time.

100% true! But as a future DO, at a small, new school, without a home program, without connections or strong mentorship in surgery, without LOR from academic surgeons, it is much harder to match if you don’t get yourself out there, and aways are just one way to do it.

This is why medical school admissions is so important.
We NEED FM/IM/EM docs committed to rural practice.
We NEED general and surgical sub specialty access in rural areas.
We NEED med students excited and empowered to GO INTO PRIMARY CARE.
Be the change you want to see.
But stop snubbing FM and and those who completed 1 yr of residency because you feel superior.
Patients in some rural areas are seeing volunteer nurse practitioners in a dollar general parking lot. Wake up.

My biggest rec: leave all your cards on the table. Go all in. Stay late because you want to, get there early because you’re just so excited to be there. You don’t want to look back and regret not putting in that lil extra effort.

My husband had a stroke due to a ruptured AVM, he had no insurance. The debt was unfathomable and was the last the we wanted to worry about during that time. Dr. Chino is doing amazing work.

Research is wild: you start off just excited to be a part of anything, at some point you realize what you like to do, then you start getting projects you are actually passionate about and suddenly you’re excited to analyze the data? Am I a nerd now? Yes, yes I am.

You can learn to intubate on a mannequin pretty quickly - doesn’t mean you can intubate a pt.
You can learn how to do stats analysis - doesn’t mean your conclusions are significant or beneficial.
But blaming the trainee doesn’t do anything. Fight the system not the person.
If all the MDs up and decide they want to do research, who is going to practice medicine?
I guess all the PhDs. We can learn to diagnose, intubate, and prescribe in a month, right? Like y’all do a month long research rotation and be ready to publish “groundbreaking” research.

For surgical specialties, use surgical letters. For other specialties a mix will *likely* be ok. LOR from “well known” academic physicians are more helpful if you are shooting for academic institutions. For community based programs it’s not as emphasized.

I get waves of anxiety in my chest and Monday feels way more daunting: will I happily celebrate my greatest achievement or will it launch the 2nd worst week in my life?
The other day, I had I patient who I sat with while she cried. I listened to her fears and validated her emotions.
THAT was the medicine she needed, and it was an honor and a privilege to be able to provide that to her (plus appropriate pharma).

TAKE STEP 1/2, have good LORs (in content and in format) from surgeons
Have a great personal statement, it should explain why surgery and who you are
Every program has a culture to it, find the programs where you fit in (rotating there helps with this one).
Day 1 of Ob/Gyn and my preceptor introduces me to patients in a way I have never been confident enough to do: as Student Dr. Vangeli.

Sad to be concluding my pediatrics rotation but so thankful for my incredible teachers.
Big take away: Kids are unique and resilient, they’ll hate you one second and high five you the next.
I don’t know if I’ll end up in

Final thing: Hobbies can be viewed as a method of mitigating burnout by the ACGME - showcase your hobbies, be proud of them, they are interesting and they start conversations

This is also my opinion but if I have an opportunity to talk to someone who is actively involved in making interview/rank decisions, I’m not wasting a second on “will you take DOs?”
If they never have a DO, you might be the first.
If they have DOs, they might not choose you.
It’s a weird feeling seeing a pt whose condition has worsened: as a med student it’s cool to see new things, but it also means a pt’s disease has progressed.
Being in healthcare is a privilege and I’m thankful for the patients that let me learn from them.

This looks like a soft application cap, because if you have 15 signals why would someone interview you if you didn’t signal? However, this past year I felt my signals were largely disregarded (even from “safeties”) despite having a decent # of IVs overall.

This gives programs another reason to disregard the applicant (no signal = no IV), which makes this change program benefitting rather than applicant benefitting. While applicants can still be ignored despite signaling if they don’t meet the other filter criteria used by programs.

Speaking as a past coach and current med student: if the behavior has the potential for harm -> immediate straight forward call out (ex. Student being insensitive to pt). If behavioral pattern is not helpful, but not harmful, a private conversation without ridicule is best, IMHO
Sometimes imposter syndrome is worrying that someone will judge me for having scuffed shoes. And sometimes it’s not following a twitter account because I would never have a chance there anyways.

I’ve redone some sections of Uworld for COMAT prep and I definitely remember questions and auto select the answer because of that, I’m definitely leaning towards getting Amboss
Having a ✨medically complicated✨ spouse means that the Vangeli Emergency Medical Service (NREMT certified) is activated at the drop of a hat.

And when looking at the most recent match data, DO seniors fill 29.7% of all fam med positions which is higher than MD schools at 29.2%, despite having far more MD schools. Newer schools in general will claim to fill the doctor shortage, DO schools actually do it.
The biggest takeaway from my IM rotation was that who you are surrounded by can make or break your experience. I was lucky enough to be surrounded by badass residents and OMS4s that cared about my education and interests, they made the rotation 1000% better.

Pick a QBank (Amboss, UWorld, Boards & Beyond). Begin practice M1 topic Qs now. Slowly incorporate M2 topic Qs. 6 months before planned STEP1, take NBME to check progress/areas to pull up. Make 6 month plan & stick to it. Consistency helped me be successful on STEP1.
Started my surgery rotation this week, and I have truly never been happier to be able to confidently validate that this is what I want to do forever! Absolutely loving it.

I did a rotation in January, so 1 month before rank lists were due and was advised to also signal the program to emphasize interest. I was interviewed there, did not match there. It might play into how you rank, but probably not how they rank you if they do interview you.

Same thing with “Rounding”, I had no idea what the term meant until I started rotations. It might have been obvious to other but it sure wasn’t for me.
