Interesting RCT in @NEJM about platelet transfusions prior to CVC placement in people w/ thrombocytopenia (Plt 10-50k): - higher rate of grade 2-4 bleeding w/o Plt transfusion: 11.9% vs 4.9% - difference driven by much more bleeding w/ subclavian lines https://t.co/MwlDtLPxub 1/

Scanning a patient in ICU...Stopped to trace LV. Literally, put the probe back on patient...then this...instant, solid clot. CPR started...no point. Entire heart clotted in a flash. Case courtesy Julia Lewallyn Ronin.

Pull up your bootstraps- this weeks case is a doozy... 67F post-EGD became hypoxic & brought to ICU for monitoring & NIPPV. Initially hemodynamically stable; over the next 6hrs became profoundly hypotensive requiring very high doses of multiple vasoactive agents
