Tip of the day 💡: None of this is to be construed as abnormal enhancement. When a good contrast bolus is shot through all the normal vessels light up, even in that arrowed pontomedullary and high cervical region.

#OnePediNeuroRad 🌍 Don´t forget superior vermis as target lesion of hypoxic - ischemic encephalopathy. Best seen in chronic stage as focal volume loss. I ´ ll give you a differential based only on superior cerebellar atrophy: ITPR1-Related Disorders

Tip of the day 💡: Sometimes one has to zoom in to the superior cerebellar vermis to pick up “dysfoliation”- this may be the only abnormality, and is often associated with a genetic disorder- a KIF mutation in this case. Did you appreciate the disordered folia with small cysts?

Talk about seeing red! Vitreous blood w/large subarachnoid hemorrhage--it's NOT intracranial SAH dissecting along the optic sheath It's from sudden⬆️intracranial pressure causing central retinal vein obstruction=hemorrhage, called Terson's syndrome. Poor prognosis #NeuroTwitter

Tip of the day 💡: 2 episodes of encephalopathy- first and second image 1 year apart. In a young child with recurrent attacks, especially in the context of ANEC, the mutation to check for is RANBP2. Tested positive!

Do not mistake pars intermedia cysts for pituitary adenomas. They are a common incidental finding, with simple or proteinaceous cyst content, located in between the pars distalis and pars nervosa, without mass effect or suprasellar extension. #radres #Neurorad #Neurosurgery #nsgy
