#Newpub 📖 🚨.@saketgirotra @JACCJournals . In Hospital Cardiac Arrest #IHCA incidence rates vary >20✖️ across hospitals 🏥 with only modest correlation with survival. As IHCA survival rates plateau; ⏰ to consider IHCA incidence as an additional quality metric? . 🧵(1/n).

🏥 Should IHCA incidence be a quality metric? Huge Medicare dataset shows low correlation with survival—yet nurse staffing made a difference. #JACC #cvGeri #EPeeps @saketgirotra

@JACCJournals @jones_daza @jasonwasfy 📢 Thanks for reading the 🧵!. This is a step toward making #IHCA incidence a visible, measurable, and actionable target 🎯. Questions/thoughts/feedback? Please reach out! 💬. @UTSWIMchief @UIowaCVFellows . (11/11).

🖊️In Conclusion: .With >300,000 IHCA events annually in the 🇺🇸 and survival rates plateauing recently 📈, targeting IHCA incidence offers a promising path for additional gains 💡. A shift in focus might help move the needle further on outcomes 🏥. (9/n).

🧠 Where do we go from here?. Future studies should explore novel strategies like real-time EHR alerts/AI tools for clinical deterioration 📉. These could ID patients "before" arrest—potentially "preventing" IHCAs 🛏️ & improving overall survival 📈. (8/n).

⁉️But, what about Rapid Response Teams (RRTs)? ⚡️. Most #GWTGR hospitals have RRTs, yet IHCA incidence vary widely 🛏️. 👉 Due to heterogenous RRT structure/operations. 👉 ~2/3rd IHCAs occur in ER/OR/ICU— where RRTs aren't activated, truncating their prevention potential 🚫. (7/n)

🧩 So, how do 🏥 address #IHCA incidence? . A) Capture the “population at risk” to enable incidence quantification & track improvement 🧮. B) From American Hospital Association data, ⬆️ nurse-to-bed 👩‍⚕️/ 🛏️ ratios emerge as a modifiable factor linked to better outcomes📈. (6/n)

🔍 We found >20✖️ variation in case mix–adjusted #IHCA incidence rates across hospitals 📊. Risk-standardized survival had a relatively narrower distribution📊. 📉 Only modest correlation between incidence & survival—suggesting incidence may be an independent target 🏥. (5/n)

🔑 We link #Medicare Part A data with #GWTGResuscitation at the 🏥 level to define the “population at risk”🧮—enabling incidence rate calculation📊. We adjust for acuity using hospital #casemix indices & use American Hospital Association data to assess 🏥 characteristics. (4/n)

Why care about #IHCA Incidence?. 2️⃣ decades of research improved IHCA survival, but outcomes remain grim —only 1 in 4 survive to discharge💔📉. ⏰ to revisit an old strategy: prevention > cure🧠. 🔑 challenge: prevention needs incidence data—missing from major registries📊 (3/n).

I would have liked to see #ViratKohli go out of test cricket before a packed stadium. But since that is not to be let us applaud him wherever we are. He told a generation weaned on T20 cricket that test cricket is cool and aspirational. And for that, the game owes him big time.
