Throughout my 16 years of campaigning for safer maternity care the number of times I’ve heard about medical maternity records (it’s always the crucial one’s relating to critical aspects of care) going missing - is uncanny. #coverupculture
I have never known notes go missing as much as they seem to in maternity depts after a problem occurs. Medical notes elsewhere rarely get lost. It’s v odd.
