Thank you @InsideTimeUK for sharing our complaints process flow chart for prisoners in August's newspaper. We hope that prisoners can keep this flow chart and it will help them navigate the internal complaints process before coming to us.

Last week @PPOmbudsman Adrian Usher visited @HMPWarrenHill and @HMPHollesleyBay prisons. Both visits were positive - Adrian had the opportunity to speak with both staff and prisoners separately and he was impressed by the accommodation at Hollesley Bay.

Today we hosted a roundtable on property complaints in prisons to share learning from our latest Learning Lessons Bulletin with @hmpps Governors and Heads of Business Hubs. The useful discussion sparked conversation on some helpful changes that could improve the current system.

Behind every complaint investigation is a prisoner for whom a swift resolution is incredibly important. We never lose sight of the fact our work has a real impact on the lives of individual people, so we pushed back 170 complaints to the establishments to reinvestigate properly.

A major focus for our complaints team last year was the launch of IPCI Ambassadors in 47 prisons, with volunteer prisoners guiding others to know how and when to contact IPCI. Getting things sorted quickly and locally is the best outcome when things have gone wrong for prisoners.

We are acutely aware that when a prisoner dies, bereaved families want answers to their questions quickly. We started investigations into 486 deaths last year, a 35% increase. We are concerned that 100 of these were self-inflicted deaths. This is too many.

.@HMIPrisonsnews annual report stated 39% of prisoners told them it was easy to get drugs. Drones are becoming more sophisticated and being used to deliver drugs into prison. The prison service need to be relentless in their drive to prevent synthetic opioids getting into prison.

As in previous years, property was the most common category of complaints we completed in 2024 to 2025 (35%). We repeatedly see issues where staff fail to handle property, and we have found that prisons are insufficiently motivated to improve property practices.
