DOC provides minimal and inconsistent data to the public, oversight bodies, and family members regarding the circumstances of deaths in custody.

DOC also did not appear to engage in regular agency-wide analyses of the factors contributing to deaths in custody to identify changes to staffing, training, or policies needed to reduce them and provide accountability.
