Great thread, with which I largely agree. ACPs aren’t”associates” for anything as some conflate one group with another. They’re not doctors and don’t want to be as I’ve stated many times. They’re vital for healthcare. It’s not “smug” to stand up for them. #LoveYourACPs

I may well regret this, but for what they are worth here are my thoughts on the ACP debate in one thread. Please do read them all, remember I am just a consultant in EM with no RCEM responsibility and hopefully my passion for EM is evident after a quick google of my name.

With the new proposals will prescribing law change to allow PIP’s to prescribe these meds (namely fentanyl)? Also, why is ketamine not on this list - I feel that we would have everything we would need from a critical care perspective. @ParamedicsUK @thepharmamedic

More than a million people aged 60 and over faced waits of 12 hours or more in England’s A&E’s last year – new analysis from RCEM reveals. And shockingly, the older a person is, the more likely they are to experience a long stay in the ED. It comes following the publication of

These findings probably highlight the need for choice. Paramedics, as autonomous practitioners, should be able to choose the right analgesia based upon the clinical need for a particular agent. The next study should perhaps look at selection of analgesia based upon patient needs

We have a population that is changing, and we can't keep doing the same thing. Bringing someone to the ED is an intervention that may not be in their best interests. We need to get better for them and for us. #eusem2024

I don’t know how much more the system can take, 10 crash bells a shift to a waiting area holding collapsing patients who should be in higher acuity areas, elderly cold and hungry without staff to care. We’re past change, it’s time to reform emergency care. @weds treeting

A&E doctors ‘not confident’ heading into winter - almost all worried patient safety is at risk. Almost nine out of 10 UK A&E clinicians who responded to RCEM's survey aren’t confident their EDs will cope well this winter. Full article & survey results: https://t.co/UYE9cIdUhN

Under pressure, you don’t rise to the occasion, you fall to the level of your training. When mission performance is critical, simulation should be close to reality. Train as a team. Use human ‘patients’. Simulate real physiology & pathology. Create pressure & immersion.
