A couple things that puzzle me on the most recent AP Poll: Two loss Notre Dame: Ranked 13th. Those two losses: No.2 Miami and No. 4 Texas A&M. Two loss Illinois: Unranked. Their two losses: No. 1 Ohio State and No. 3 Indiana. One loss USC: No. 21. That one loss: #Illinois.

@DrDiGiorgio As an outpatient IM/primary care physician who is on the receiving end of hospital notes, the pattern that gets followed: copy and paste the ER cc and HPI. Keep that same note in every subsequent hospital progress note. (don't update with changes in symptoms, etc). Add a table of
