(4/4) A bit more cement and done. The main principle for treatment of hinge-fractures (DISH, ank spond) is stabilization, primarily from compressive forces. Cement from top of T9 to bottom of T10 accomplishes this. Close follow-up to ensure stability and tx add'l fractures.

(3/) After initial cement from these accesses, the air filled fracture clefts are filled, but right side T10 is bare. Osteophyte blob = 👍🏼 Old accesses removed, new right transpedicular access with standard craniocaudal angulation. Moar curette. Curette = 🐐

(1/) Left parapedicular/costovertebral access to center of T10, right transpedicular access with cranial trajectory to T9-10 disc space and T9 inf endplate. T10 balloon faced pressure from the sclerotic but unhealed endplate.

Sometimes controlled extravasation is beneficial. T10 and recurrent T9 fracture s/p T8,9 kypho previously. Fracture morphology complicated by DISH. Transpedicular disc access for T9 kypho and discoplasty to consolidate osteophyte and endplate. Ugly, but effective. #disasterplasty

1. off-label device use. 2. paravertebral extrav at T6 and T7: ugly but clinically insignificant. Usually leak from a fracture line extending to the lateral cortex. Catch it, slow down, let it plug. Proceed. Same for small vein at T6.

Curved needles and cement cannulas are helpful for targeting focal fracture clefts. Small T8 superior endplate cleft after otherwise sufficient kypho at OSH was not amenable to pedicular access. Transdiscal access from above made for an easy fix.

Clumping of the nerve roots in arachnoiditis is very commonly missed even in patients like this with chronic leg pain after trauma and prior surgery. keep an eye out for abnormal distribution and clumping of nerve roots in the cauda equina.

Yet more support for large volume RFA of the SI joints statistically, significantly improving patient’s pain function & quality of life out to one yr for patients suffering from SI joint dysfunction. RFA provided benefit regardless of prior standard medical mgt treatments.
