We had great results using the electrogramas de desfibrilador to guide HPSD ablation of fascicular substrate for >90% freedom of VF at one year. Full paper in press @JICE_EP

Our star EP Fellow Rob Kerley (winner of the VT Innovation Award) wrote up our experience using ICD electrograms to target fascicular substrate to ablate VF. When combined with a HPSD RF we observed very favorable outcomes. Free read from JICE https://t.co/B1YZJnARLi

Please look at Gene Quinn et al paper. The problem and this is warning. You can’t unsee the massive variance in stroke risk at a set CHADSVASC score. https://t.co/n2JKHLifnx Read paper. Save it. CHADSVASC calculator = folly

@drjohnm Issue may in part be chads and chads vasc sig. overestimate risk of stroke. So question really becomes who needed ac in the first place.

Cardiac MRI with #ADAS3D mapping helped delineate the VT isthmus in a patient with idiopathic triglyceride deposit cardiomyovasculopathy (TGCV) — guiding successful ablation and expanding understanding of this rare disease. 👏 Thanks to Dr. @KamakuraTsukasa and team

Early-onset AF (<50) is often the first sign of inherited cardiomyopathy/channelopathy. Genetic testing = high yield ➜ impact on treatment decisions 🫀 #AtrialFibrillation #Genomics #PrecisionMedicine
Genetic testing in early-onset atrial fibrillation uncovers a high rate of pathogenic variants and changes clinical care. Read more in #EHJ. https://t.co/GSXwUGbc7E

I tell my patients who come see us at our Haverty Cardiometabolic Center, “the a.fib is the tip of the iceberg, and what’s below the surface is driven mostly by the metabolic disease of obesity.” - which drives the HTN, DM, inflammation, HFpEF, etc.
Among patients with #AtrialFibrillation, elevated BMI, and 1 or more additional cardiometabolic risk factors, aggressive risk factor management reduced arrhythmia recurrence over the 12-month period after catheter ablation. https://t.co/2SBgFqTLei

Patient with intramural outflow tract PVCs. Failed sequential PVC ablation on either side. Bipolar ablation performed with Stereotaxis catheter via transseptal at earliest in LVOT &manual cath in RVOT w/ elimination after 30s. No PVCs at 3 months. #BipolarAblation #EPeeps

Right now, we have limited input into PFA delivery. However, we can modulate depth of lesions by altering inter-pulse delays and cycle length of pulses. This might allow for deeper lesions for PFA treatment of ventricular arrhythmias. @utedrow

PVI+ is the one area where RCTs have been unable to supercede anecdotes – because operator bias is so powerful. Only two randomized trials have shown efficacy beyond PVI (CONVERGE and VENUS), yet only a minority have adopted. And multiple PWI studies show no systematic benefit
What’s the obsession with PWI in AF ablation? It may appear “safe & easy” @drjohnm BUT no randomised evidence & there is downside #TWIC Oct 10

Outcome of Pentaspline PFA outside PVs 👇 Acute success does not necessarily translate into long-term success.

Great talk @DJ_Lakkireddy on important limitations of PFA outside of the veins. Single digit long term durability in La Fazia et al. Dual energy sources can’t come soon enough for redo ablations. There need not be discussions with current PFA iterations for VT.
