when I use etomidate, I generally add fentanyl (analgesia) & midaz (prolongs sedation >5 min & reduces risk of awareness) I wonder what the BP difference is between ketamine vs an etomidate-midaz-fent cocktail?? ๐ค my guess would be: not much donโt use pain as a pressor
@SkylerLentz my preference during ketamine shortage is a cocktail ๐ธ 1) 5 mg midaz 2) 50 mcg fentanyl 3) roc 4) etomidate give them all in sequence w/o flushes inbetween & they all hit at about the same time midaz/fent keeps patient amnestic & comfortable when the etomidate burns off

in thyroid storm, treating sinus tach with a beta-blocker will occasionally cause death many patients have reduced LVEF & compensatory tachycardia (borderline cardiogenic shock) beta-blockers push them over the edge what data supports B-bl in thyroid storm? why do this?

๐ก OMI paradigm ๐ก is essentialโ The patient came after 10 minutes since the onset of a crushing retrosternal chest pain during rest. Cathlab denied admission with these ECGs (no STEMI for them). The young colleague was aghast in her shift just like me today seeing these ECGs.

One of the most overlooked forces in healthcare is Goodhartโs Law: โWhen a measure becomes a target, it ceases to be a good measure.โ In organizational behavior, this creates metric myopia - where people optimize the number, not the outcome. Behavioral economists call it perverse

@TheECGMedic Been saying for a while now that de Winter T waves are not restricted to the praecordial leads. I believe we've had this talk before as well!

Wide QRS complexes may be PVCs or PACs with aberrant conduction. If you see a typical bundle branch block pattern (red box = RBBB+LAFB; purple box = LBBB), look for preceding P waves (gray arrows). Therefore, these are PACs with alternating BBB patterns.
