ACL injuries happen in pitchers too. The problem? There’s no clear protocol on when and how to return them to the mound. Here’s what I look at when helping pitchers return to the mound after ACL surgery . Which knee?.- Block leg: must tolerate rapid decel forces without leaking

“The Standard Is The Standard.” One of our UCL-repair athletes walked in sporting that line on a high-school tee, perfectly capturing our rehab mission: get back to throwing not merely pain-free, but stronger, more powerful, and more durable than the day the ligament tore.

The #1 mistake in Tommy John rehab is delayed loading for arbitrarily conservative reasons. Why wait 3 weeks to introduce a stimulus they can handle TODAY?. Delayed loading leads to rushed end stages and vulnerability when it matters most. Progress by criteria, not the calendar.

The vast majority of the time, your [insert muscle/body part] tightness is not a flexibility/mobility issue. It’s a weakness and exceeding tissue capacity issue. The tightness is communication from the body that you are currently red-lining the capacity of the tissue to handle.

Rehab providers 🗣️. Muscle size doesn’t alter the load needed for strength or RFD gains. Endless 3 × 15-20+ rotator-cuff reps with a flimsy band won’t bullet-proof a shoulder, progress the load and velocity if you want real adaptation.

How many weeks into this athlete’s return-to-throw are we post-TJ recon?. 8? 11? 16? 24?. He’s Week 11, Day 3. That might surprise you, but working with high-level throwers often requires a different approach than your standard RTT. Here’s my thought process:.🧵
