It's Cost Improvement Programme season in NHS Trusts. Departments are scrabbling to please organisational leadership by upping their budgetary efficiency. Let's use the Job Evaluation Handbook and save some money!. TL;DR: you're overpaying your PAs. Long 🧵 incoming. 1/.

Interesting post-dinner discussion this evening around ward rounds inspired by @DrLKVaughan's thread from yesterday - listening to two very different generations of doctor, one of whom could not imagine a world without a firm system, and the other having never experienced this šŸ¤”.

patient, as triadic consultation is also standard for us. We have 46 inpatient beds. Our system isn't funded to provide safe and sufficient care, never mind educational excellence, and it's VERY frustrating. Still, it's lovely to dream. Also, LOL:

the pts ahead of actually seeing the pts. But this requires pre-rounding, and it's slow. If the alternative is a scribe/noctor, then the system will need to accommodate the fact that consultants can only see 12-15 pts per day on their own safely. 8/

Dr Blackwell's ward rounds sound like an excellent learning experience, and I'd love to be able to deliver them. I work on a single "consultant of the week" model in a specialty committed to consultant-delivered care and we expect the primary carer to be present with the. 1/.

Last night covering the strikes. Solidarity persists department-wide for our excellent residents. Meanwhile, the crash bleeps are down, so I get a crash walkie talkie tonight and I'm illogically giddy at the thought yet terrified I'm going to leave it lying around somewhere 😬

Pleased to report that the night team I am currently covering have chosen to strike in their entirety, and this particular consultant can use a computer, so no significant patient safety issues. What an absolute embarrassment. Imagine working for this lot.

Due to the failure to train consultants on a new EPR system, lack of planning and action taken by the Northern General in Sheffield, we are forced to accept a derogation for 3x resident doctors for tonight's night shift (26th July) for Acute Medicine. Protecting patient safety.

Full solidarity with our striking residents. The job is not 20% easier than it was in 2008. The nation's average salaries have recovered since then. No reason why a job this mentally and emotionally challenging deserves different. #PayRestorationNow.#ResidentDoctorStrike
