šÆ. Helpless-sounding authorities & professionals plead: āItās like trying to find a needle in a haystack.ā . Wrong. (Difficult, yes; but that could actually be done.). Itās more like trying to find a single piece of hay (which looks just like all the others) in a haystack.
Fact check: It is statistically impossible to predict school shooters without hundreds of thousands of false positives for every prediction. (For the statistically minded, this is about Bayes theorem and low base rates).

This gentle man lives on the streets near me, has for years. Gets lots of sunlight, exercise, & good food (near-expired produce fr grocer, whose patio bench he sleeps on). Refuses approach by would-be helpers lest, we interrupt his talk w what he hears (that may need nurturing.)
Iāve met so many who hear what others donāt, see what others donāt, feel more deeply than most - and were told by mental health professionals their brains were broken, not that they had a gift to be nurtured. Itās hard to express the sheer contempt I feel toward those āexperts.ā.

It surely can be stigmatizing, for a subset of people (maybe a majority)⦠this only elevates the crucial importance of acknowledging the subset of emotional suffering that IS mental illness; denying or ignoring that prospect results in tragic degrees of preventable suffering.
The stigmatising idea that people in emotional pain may have brains (or parts of brains) that are dysfunctional, defective, imbalanced &/or disordered has become part of our zeitgeist, rendering our relationship to suffering more fatalistic, fearful & tortured.

Psychiatry is swimming upstream. Thorough treatment has all but disappeared. Little depth psychotherapy is available, robust inpatient extended stays no longer available (most IP units are just management/supervision mills. Suicide/suicidality track most strongly (& inversely).
Last week, I was called "unduly pessimistic" about psychiatry. Is it "unduly"?. Here are some facts: Then ā Now.⢠Suicides: 30,622 ā 49,316 ā¬ļø61%.⢠Psychiatric disability: 2,053,173 ā 3,011,707 ā¬ļø47%.⢠Teens considering suicide: 14% ā 20% ā¬ļø43%. Yet.⢠Adults taking

As inpatient psychiatrist mid-90s I spent 1-2 hrs ev day on calls like this, pleading for xtra days for psychotic & suicidal patients. Once asked how I knew pt was suicidal, said āhe bought a gun.ā .Reviewer: āWas it loaded?ā.I knew it would hit the rest of medicine eventually.
Iām sharing an actual recent peer-to-peer call that shows what physicians and patients face when trying to get a surgery approved. This call felt as absurd as it sounds. The peer-to-peer call was to advocate for surgery to prevent and treat lymphedema for a patient with breast
