6/n.This emerging view connects neuroscience, psychology, and clinical care. Predictive coding may hold the key to understanding—and treating—chronic pain.

5/n.Our findings align with predictive processing theories: the brain expects pain, and that expectation shapes what we feel. Placebo may work by updating these predictions.

4/n In pain, the prediction of pain (“I’m in pain”) and bottom-up disinhibition both enhance pain perception. In placebo, the prediction of relief (“This pill will help”) and bottom-up suppression work together to reduce it. Predictive mechanisms modulate both experiences.

3/n.We examine top-down, bottom-up, and intrinsic effective connectivity in a minimal cortical pain network .Findings?.🔹 Top-down connectivity becomes more inhibitory in both chronic pain and placebo.🔹 Bottom-up becomes excitatory in pain, inhibitory in placebo.

2/n.Pain is a story the brain tells. It’s not just sensation—it’s belief, expectation, prediction, attention and sensation. We ask: how does the brain create pain—and unmake it during placebo?.
