Today I have done my 100th caesarean section (CS) as the lead surgeon (not counting the numerous others I have done as an assistant).Thankfully, all procedures have had a good outcome for both the mother and the newborn. Looking forward to more milestones.

A Caesarean Section (CS) is the delivery of a newborn through a surgical incision made in the abdomen and another incision made in the uterus. The abdominal incision can either be vertical or horizontal. The horizontal incision is the most commonly used.

With every further scar one gets, there is increased risks of complications. Therefore, depending on your facility the recommended maximum can be between 3 and 4. Not because one cannot deliver again. Simply because of the increased risks that come with more scars.

Once in theatre, a patient is put under either General(GA) or Spinal (SA) Anesthesia. GA numbs the entire body and the patient is not awake as opposed to SA which only numbs the lower body and patient is awake. SA is preferred as it poses lower risks to the mother and the newborn

To prepare for a CS, your doctor will explain the procedure to you and you will sign an INFORMED CONSENT. One is required to have fasted for 6 hours unless it is an emergency CS. You should notify your doctor of any conditions or allergies you have & medications you are taking.

1.Resume eating 6 hours after the procedure or when bowel sounds are heard (if GA was used)
2. Begin walking 6 hours after procedure-helps prevent thromboembolism
3.Remove urinary catheter once patient is ambulating

4. Macrosomia - if the expected fetal weight is above 4.5kg, a CS is done as it may lead to dystocia (difficult labour) and birth injuries.
5. Cord prolapse - this is when the umbilical cord presents before the fetus. If constricted it leads to hypoxia and fetal death.
