As a med student at the VA the med team couldn’t figure out why a patient’s hands were turning such a dark bronze color. Nothing they thought of made sense. During the daily 8 hour misery rounds, I discovered a bottle of jurgens bronzing lotion on his counter. Mystery solved.

I graduated from residency in 2014. All H&Ps/consult notes/progress notes/orders were done by hand. We also had to dictate the H&Ps/consult notes. My hand would be so tired by the end of a night on call. Anyone else have to hand write AND dictate notes?

If it were me, I’d nail the tibia and use 3 distal lockers and maybe use a tibial guidewire to stabilize the fibula if worried about stability. I’m just a slow joint surgeon though.

So I made a google form that I log info on all my joint cases. It automatically dumps into a google sheets file.m w formulas so I can mine data. Next step is to figure out how to get it to email out a custom report on a set interval.

Fun case. Had a patient present to me with significant flexion instability. She experienced significant difficulties with ADLs. One prior revision (where the suture anchors came from). On exam the instability with fairly isolated to the flexion space. Extension space was just OK.

My biggest piece of advice is to not accept poor exposure. With adequate exposure it is pretty difficult to perforate. You should be able if something is wrong by the ease of broaching and the angle the broach is sitting in the femur. If there’s and question, stop and take a shot

What sucked was making sure I was prepared for morning conference post-call. All of our conferences were taught via Socratic method and there was as no hiding. You really didn’t want to be the person that had to say “I don’t know”.

If you operate, you’re going to end up doing a# partial patellectomy. And an irrigation and debridement. And another irrigation and debridement. If the extensor lag is not significant, cast with frequent cast changes.
