North Track and Track have managed to reduced the average time taken to list a patient for care by 34% in less than a month through implementing Standard Work. PDSA & new ideas keep pouring in from the team and they are gearing up for their next challenge. @KatyMorrisQI @mpftnhs

Teeth, glasses, washed, dressed, out of bed, food and fluids within reach, independent taking meds and toilet use. Simple, you’d think. Not so much. I had to remind the ward twice, whilst pointing at the #endPJParalysis notice board next to the nurses station.
Bob, 70, is admitted tonight. He has a medical condition,lives alone+is fairly independent. Would your area support Bob to mobilise over the weekend even if there has not been any physio assessment?.Would your board round on Mon ask how mobile Bob had been?.#endPJparalysis
