Agree …multidisciplinary early. Establishment of early dialogue helps relationship building and is needed as said especially for refractory disease and #perianal involvement.

If no desire for IPAA then talk about what to do with rectum long term. Okay to leave if regular surveillance and not symptomatic. Completion protectomy if neoplasia or having to treat the proctitis.

Important topic in pouch patients especially with variability in surveillance guidelines…highly caution a redo in the setting of cuff dysplasia. Patient should at least have EUA with extensive biopsies of the cuff.
