Seems only Elon is allowed to reply to sycophancy. BTW, @elonmusk leaving DOGE is a sign of his failure, it’s delivered a lot of hot air, but we are in a climate crisis so we really don’t need more of that.
As @elonmusk leaves the Trump administration, let’s salute him on his work on DOGE. DOGE has been a great idea from the beginning. Big government is a serious problem. South Africa needs something like DOGE. Our government is too big and should be cut.

In SA when diagnosis awareness was low, & targeted only the extreme hyperactives. Inattentives were unheard of. Both onset age AND impairment definition creates the mess we’re in as it renders this professional subjectivity. We have so much stigma already - help, don’t hurt.

Why under-diagnose? Surely only diagnosing when an accurate diagnosis is possible would make sense. I’d argue that this notion that a diagnosis carries a stigma is that which is perpetuating the stigma. Do we start to see why the antis have it so easy,because this is a platter
Why I routinely under-diagnose: 1)Initial impressions often wrong 2)We see patients on worst day- doesnt extrapolate to future 3)Easy to later up diagnose 4)Hard to erase wrong label 5)Harms of overdiagnosis far worse- stigma/meds 6)I wish all diagnosis could be written in pencil

At the risk of commenting on a closed session, @AllenFrancesMD, as an avid user of AI the one quality AI does not possess, and may never, is empathy. It understands at a logical level, has access to knowledge no single therapist can fathom, but it cannot feel or emote.
I've spoken to many who use ChatGPT therapist- including therapists. All are amazed at AI: 1)"human" quality 2)empathy 3)flexibility 4)education skills 5)accessibility 6)convenience 7)cost I think healthier patients will prefer AI/Human edge only for more difficult patients.
