Please make it stop - 10 unique patients for fMRI and/or tractography in 1 week is too much!.

via @NYTimes “A lot of guidelines may seem reasonable when considered in isolation,” Dr. Johansson said. “But the cumulative burden of all guideline recommendations combined is absurd.” The same thing can be said about many other dimensions of healthcare.

Some doctors say that however reasonable guidelines may seem, their cumulative burden causes “constant frustration” to medical practice.

Even at $1500 a month probably far more cost effective than any anti-amyloid drugs at preventing or delaying dementia!.

I wrote about the new anti-obesity drugs—semaglutide (Wegovy) and tirzepatide (Mounjaro)—that represent one of the most important breakthroughs in medicine.

The end? “I know of six critical systems (…) which no longer have any engineers,” a former employee said. "There is no longer even a skeleton crew manning the system. It will continue to coast until it runs into something, and then it will stop.”

Exclamation Mark. Possibly comes from the Latin phrase "io", which was an expression of joy. 14th century Medieval writers placed this at the end of sentences for emphasis. Eventually the I was placed above the O and it became an !. Got its current name in 1839.

Word has a really really easy way to label lines of text - I don’t understand why journals don’t require it. This legacy thing just adds time for busy volunteers (aka reviewers!) to locate their comments.

No matter how well caffeinated I am, there is always one silly error that slips by me in the proof - I guess I should just embrace “wabi-sabi”?

In Louisville (2011) I thought… no way (as future neurorad) I’ll need this, but might as well be an “authorized user” and take those extra questions… today I did my first FDG injection for pet-mri….

I see this all the time. My point - if you are lucky you will see the tip of an iceberg (IHI, HS, MTS) but don’t delude yourself that MRI alone is enough!.

Extensive areas of cortical FDG hypometabolism involving insula, opercular and orbitofrontal cortex suggest broad seizure network.

Hmmm… the amygdala is NOT involved? Maybe we should check the FDG PET?

When you push the FA threshold too hard you get the “interhemispheric tract”… some people suggest this tract has 5 parts including the above plus “falx cerebri fasciculus” Parts I, II, III and V… although I’ve only ever seen that parcellation published in healthy controls ;)

Can we please just abandon “dorsal-ventral” and “cranial-caudal” in human neuroanatomy, maybe even for names of structures… I get historical issue, but it’s confusing!.
