Final comment: this is not an attack on clinicians. Most clinical docs I've met in 35 years have been superb with patients and really want to do the right thing. This critique is of the "etiology"-based system that uses a false gold standard (MDD).

PS: neither positive "autoimmune serologies" nor positive "exposures" prove CTD or HP as etiologies of fibrosing ILDs. Look up the phrase: post hoc ergo propter hoc. MDD is a game in which the clinician is both the player and the referee.

As long as we unquestioningly accept the false gold standard of "multidisciplinary discussion" where the clinician always gets the final word on "etiology", radiologists and pathologists are doomed to always be "wrong". This game is rigged, folks!.

Imaging = "typical fibrotic hypersensitivity pneumonitis" . My take:.1. Those who described "pathology of fHP" using imaging as the standard should weep.2. fHP is a hot mess.3. Assignment of "etiology" in fibrosing ILDs is a subjective fool's errand .
