Reasonable adjustments like: .- Visiting a hospice before an appt / stay .- adaptations to home to enable somebody to stay there (moving to ground floor/additional care) .- skills to break bad news and support discussion of end of life care .- MDT working with LD teams & pal care.
Ensure ALL staff groups have an awareness that people with a learning disability may require reasonable adjustments to achieve a good end of life care experience 🙌.

How are we still seeing ‘learning disability’ / ‘Down’s syndrome’ for a reason on a DNACPR form .People do not die from having a learning disability . Death may occur as a consequence of co-occurring physical disorders and serious health events. a learning disability isn’t fatal.

Learning disability annual health checks are still important if somebody has a life limiting diagnosis. They help to find other health problems early & identify other possible unmet health needs.
